Rehab Resources is proud to provide therapy services for the Dodge County Birth to 3 Program. Through this program, we work directly with children and their caregivers in their natural environment, typically their home or day care. As a child’s parent or caregiver, we value your expertise in knowing your child best. As experts in our field, we work with you to develop activities and strategies to work toward meeting your child’s goals.
In the Birth to Three Program children must have a developmental delay of at least 25% in one or more area(s) as noted through the Birth to Three Evaluation or other records or a diagnosis with a high probability of a developmental delay, or delays in achieving significant milestones.
Eligibility is not based on income or on income guidelines. The program can assess the child’s developmental ability to:
- Develop relationships
- Express and regulate emotions
- Learn and play
- Move, see, and hear
- Perform basic self-care skills
How to Apply
To refer a child to the Dodge County Birth to Three program, contact the Dodge County program coordinator at (920) 887-4282 or visit the Dodge County Birth to Three website. Parents, pediatricians, nurses, schools, and day care providers may all make referrals. Typical Information that is requested at the time of the referral include:
- Child’s name, address, and date of birth
- Name of parents / caregivers, address, and phone numbers
- Name of physician
- Reasons they are concerned about the child
